How to Make a Nomination by Fax
- Download and fill out the interactive nomination form online (Adobe Reader version 7.0 or higher is required; available free at www.adobe.com). Type your answers directly into the form.
- Print two copies of the completed nomination form (one to send to FAAN and one to keep; you cannot save the information directly into the form).
- Type your responses to the questions. Use a 12-point font and 1-inch margins. Limit to 1 page per answer. (All answers must be typed.)
- Type your name, the name of the person you are nominating, and “MCF Awards Nomination” in the upper left-hand corner of the document.
- Print your responses to the questions.
- Fax the completed nomination form and your responses to the questions to (703) 691-2713.
