How to Make a Nomination by E-mail
Nomination Form
- Download and fill out the interactive nomination form online (Adobe Reader version 7.0 or higher is required; available free at www.adobe.com).
- Print a copy of the completed nomination form for yourself, and click “Submit.” (You cannot save the information directly into the form.)
- When you click “Submit,” you will be guided through the process of submitting the data from your form via e-mail. The information you type into the form will be extracted and saved as an .xml file.
Questions
- Type your responses to the questions. Use a 12-point font and 1-inch margins. Limit to 1 page per answer. (All answers must be typed.)
- Type your name, the name of the person you are nominating, and “MCF Awards Nomination” in the upper left-hand corner of the document.
- Save your responses as a Word (.doc) file, and name the file as LastName.FirstName of the person being nominated. Note: Responses to questions copied into the body of an e-mail will not be accepted.
E-mail the application data file (.xml) and your responses (Word file, .doc) to faan@foodallergy.org. Please put “MCF Awards Nomination” in the subject line, and include your name and the name of the person you are nominating in the body of your e-mail.
