Mariel C. Furlong Awards for Making a Difference
 Each year, FAAN recognizes individuals or organizations that have provided outstanding service or have made significant contributions to food allergy awareness, education, outreach or advocacy. The awards were named in honor of FAAN's founder's daughter, who has since outgrown her food allergies.
Do you know a person or group that has done an outstanding job in educating others about food allergies? Or someone who has volunteered his or her personal or professional time to create a food allergy awareness program? How about someone who has made managing food allergies easier for you and your family, or for anyone else you know? Here is your opportunity to show those who have gone above and beyond the call of duty to make a difference in the lives of those with food allergies how much you appreciate this hard work and support. Nominate the person or group for a Mariel C. Furlong Award for Making a Difference (MCF Award).
You may nominate your child’s doctor, a teacher, a friend of the family, or even a group of people who have collectively helped make a difference. The nominee’s contribution may have involved a single significant event or an accumulation of effort over time. Sorry, family members are excluded. However, FAAN does have a special award just for grandparents.
MCF Award recipients are recognized for their involvement in increasing awareness and education associated with food allergies in four categories:
- Community Service (e.g., individual, civic organization, support group leader)
- School (e.g., school nurse, principal, teacher, school district, school staff, food service staff, school board member)
- Health Professional (e.g., dietitian, doctor, physician’s assistant, nurse, allergist, food scientist, researcher)
- Food Industry (e.g., food manufacturer, food service company, restaurant, grocery store)
The winners of the Mariel C. Furlong Awards for Making a Difference will be announced at FAAN’s annual Food Allergy Conferences in 2010.
How to Make a Nomination
To make a nomination for the Mariel C. Furlong Awards for Making a Difference, you must complete a nomination form and answer questions following the instructions below. You may make multiple nominations, but you are limited to one nomination per category. If you are entering a nomination in more than one category, please use a separate form for each.
All nomination forms and short answer questions must be typed. Nominations must be postmarked, faxed, or e-mailed by 11:59 p.m. ET, Friday, Jan. 15, 2010.
The nomination form requires Adobe Reader version 7.0 or higher is required; available free at www.adobe.com.
